Access to Health Records & Consumer Protections

In accordance with Texas House Bill 4224 and Texas Health & Safety Code, Chapter 181, Lotus Rayne, PLLC provides the following information regarding client rights related to access to health records and available consumer protections.

Requesting Access to Health Records

Clients have the right to request access to their health care records. Requests must be submitted in writing to Lotus Rayne, PLLC.

Written requests may be submitted by:

• Requesting and completing a written records request form

All requests for records will be processed in accordance with applicable state and federal laws, including timelines, permitted fees, and allowable limitations on disclosure.

Filing a Consumer Complaint

Clients have the right to file a consumer complaint with the Office of the Texas Attorney General if they believe their rights regarding access to health records have been violated.
